Opthalmoplegia

Weakness or paralysis of the muscles of the eye.

 

Patient 1:

This is a young woman who presented with right eye pain, tearing, and diplopia after sustaining trauma to the eye. She was found to have acute upward and downward gaze palsies of the right eye. Cross-sectional imaging showed a right orbital floor fracture with entrapment of the inferior rectus muscles.

Patient 2:

This is an older man with heavy alcohol use who presented with progressive confusion, ataxia, and a left lateral gaze palsy, consistent with Wernicke’s encephalopathy. He improved dramatically with thiamine replacement therapy.
